Permission & Agreement Forms
Please click on the links below to access a number of permission and agreement forms that require your signature, as well as information about privacy and use of photographs.
We record the permissions that parents give for their child electronically. Parents are sent the link to record their permission preferences at the start of each school year.
If your child has joins us at any other point in the school year, you should receive a link to the electronic form from the school office. If you have any problems finding this link, please email email@example.com and ask for the link to our General Permission Form 2021/22.
You can also change the permissions that we have for your child at any time. If you would like to re-submit the permission form for your child, please also email the school office for a link to our General Permission Form 2021/22.
